How can friends & family access my health information?

We will only disclose your health information to individuals you specify on your HIPAA release form. Your health information may also be accessed through your RXNT.com patient portal. Access to your portal is controlled by you; we do not provide others with that information.

How can I have my health records sent to someone?

You may request your health records to be sent to other agencies by contacting our office. If you wish for paper copes to be sent, a fee may be charged.

How can I reschedule my appointment?
